Missouri School Immunization Requirements

All students must present documentation of month, day, and year of each immunization before they attend school.•
All immunizations must be up-to-date before students are permitted to attend classes.•
For children beginning kindergarten during or after the 2011-12 school year, required immunizations should be administered according to the current schedule.•
To remain in school, students "in progress" must have an "in progress" form on file and must receive immunizations as soon as they become due.•
Religious and medical exemptions are allowed. The appropriate exemption card must be on file.Grades /Immunizations

1. DTaP Last dose on or after fourth (4th) birthday and last dose of pertussis before seventh (7th) birthday. /Tdap booster before 8th grade if over 2 yrs.
Maximum needed: six (6) doses.
2. Last dose on or after fourth (4th) birthday. If a combination of IPV/OPV is received, four (4) doses
are required. Maximum needed: four (4) doses.
3. A statement signed by parent/guardian or physician indicating the month and year the child had chickenpox disease.
4. Students who are 11-15 years of age may receive two doses adult formulation Merck Recombivax
Hepatitis B vaccine on the following schedule:
Dose 1, initial visit Dose 2, 4-6 months after Dose 1
5. Td booster required ten (10) years after last dose of DTaP, DTP, DT, or Td.
